How the School-Based Health Center Works:

We are happy to be entering our forth year of school-based health services offered by PrimaryPlus.  Our team will continue to work with the school systems to grow and extend services to families throughout the region. PrimaryPlus currently offers School- Based Health services to Bracken County and Augusta Independent school in Ky and to Ripley Union Lewis Huntington schools in Ohio. After completing the School-Based Health registration forms, your child can be seen by our Family Health Nurse Practitioner during school hours for minor sickness such as earache, throat infection, eye infection, skin irritation, cold, runny nose, etc.

Note: (parents can choose to be present either in person or Telehealth).

Parents can also call to schedule an appointment for themselves or their child when needed.

 

How the School-Based Dental Center Works:

PrimaryPlus Dental services are also offered in our Ripley School-Based Health and Bracken Co School-Based Health locations. . Dental services include basic dental exams, cleanings, x-ray and some restorative procedures. We want to ensure happy, health, beautiful smiles for the children of this community.

Note: If your child has a dental home that includes preventative care every 6 months, you do not need to utilize the school-based dental program

Q: Will I be notified if my child needs an appointment?

A: Yes, if your child presents to the school nurse, the school nurse would first assess the child, contact the parent, and if all consents are in place and the parent/guardian agrees for the child to be seen, the nurse will then work with PrimaryPlus to get the child an appointment with the school-based program. Note: If all school-based health consent forms are signed and the parent cannot be reached, a visit can still take place. Every attempt will be made to contact the parent after the visit to inform them of the care plan. A care summary can also be provided.

Q: What if my child is already a patient of PrimaryPlus?

A: If your child is already a PrimaryPlus patient whether at our Bracken site, Kid Care or other locations, GREAT! We are happy to continue to serve you. We will still need the parent/guardian to fill out the school-based consent forms. However, the great news is—if your child has been seen at a PrimaryPlus location, our team will have access to those medical records--PrimaryPlus’ Electronic Medical Records are all connected.

Q: What if my child is NOT a patient of PrimaryPlus?

A: If you currently have healthcare providers outside of the PrimaryPlus network, your child is still welcome to utilize the school-based health services—consent forms will need to be completed. We wish to stress that school-based health is not designed to take the place of your child’s regular pediatrician, primary care provider or dentist. We value and respect our fellow healthcare providers and we will be happy to provide a care visit summary with them.

Q: Will I receive a bill for this service?

A: PrimaryPlus will bill the necessary insurance or party for the health center visits. If your insurance has a co-pay the parent/guardian will receive a statement via mail. IF YOU HAVE NO INSURANCE COVERAGE: PrimaryPlus offers a Sliding Fee Program that provides up to a 75% discount on most services (50% on dental services). This program is available to all patients--uninsured or underinsured.

Q: What is School-Based Telemedicine?

A: The PrimaryPlus-Bracken care team will use a computer to teleconference in (like zoom or Facetime) to see the patient—this is a HIPAA Compliant and secure program. We have purchased advanced equipment that uses specialty cameras and stethoscopes for the school nurse to use allowing our medical provider to see inside the ears, throat, nose, eyes, etc. plus get a blood pressure reading, pulse reading and hear heart/chest sounds—all in real time via the conference call. The PrimaryPlus medical provider will then be able to give a medical evaluation and provide proper treatment. The great part is that if a parent/guardian would like to take part in the telemedicine visit they can…all without leaving the home or the office. A link can be sent via text or email for the parent to join in on the visit! If the parent does not wish to join the telemedicine visit, the medical provider will attempt to contact the parent/guardian via phone to discuss the findings of the visit.

Q: What if medication is needed for my child?

A: Any necessary prescriptions will be sent to your child’s preferred pharmacy after the school-based visit. Again, if a parent was not part of the visit, our medical team will attempt to make contact with the parent to review the treatment plan and appointment

Q: If I completed the packet last year, do I have to complete it again?

A: Yes, school-based consents have to be updated each school year to ensure we have the most up-to-date information possible for your child. Plus, with the addition of dental services, parents can select which service they wish their child to participate.

PrimaryPlus accepts most commercial insurance plans, Medicare; All KY Medicaid Plans, OH Medicaid (CareSource & Molina) and also offers a Sliding Fee Scale program for those with an inability to pay.
