In June of 2009 the Bracken County community welcomed the opening of our PrimaryPlus-Bracken office with open arms. That “hometown” touch is very present in the Bracken office. Considering that much of the staff grew up within the community, there is a great sense of comfort and familiarity. Services within this office include family health, pediatrics, x-ray, minor procedures and an On-site Pharmacy! We are also humbled to offer our trademarked WeCare® program at this facility utilizing a team of Medical Providers, Case Managers, Pharmacists, and more to give a TEAM approach to chronic disease management.
Affordability
PrimaryPlus accepts most insurance, Medicare, all Kentucky Medicaid plans, OH Medicaid (CareSource & Molina) and offers a sliding fee scale for those that have NO insurance and/or are underinsured.
