WeCare® is composed of an amazing group of cheerleaders and coaches (aka Nurses, Medical Providers, Pharmacists, Dietitians, Certified Diabetes Educators and more) to help guide you through this journey to a HEALHTIER YOU!

The Case Managers for PrimaryPlus’ WeCare® program serve as the tour guide on the diabetes journey helping patients understand their health, the opportunities, the resources and the benefits. Their nursing background provides them with all the right tools to help you achieve optimal health. It is our goal to work with the patient on many levels to close care gaps and be a direct line of communication.

PrimaryPlus’ Registered Dietitians are excited to help educate patients on ways to have a healthy relationship with food. They provide Medical Nutrition Therapy (MNT) at most PrimaryPlus locations. They believe in helping you assess your diet--teaching you to make REALISTIC changes. These recommendations will be tailored just for you based on a variety of factors while also taking into account your current eating habits.
